Job summary

Fairview Skilled Nursing Home in New York is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure timely assessments, and support quality resident care and reimbursement. The position requires an active New York State RN license and prior MDS coordination experience in a skilled nursing facility, with knowledge of MDS 3.0, PDPM, and CMS regulations.

You will collaborate with the interdisciplinary team, monitor schedules, and assist with survey readiness and staff

Qualifications

• Current New York State RN license.
• MDS Coordinator experience in a skilled nursing facility preferred.

• Strong organizational, communication, and documentation skills.
