Qualifications

Job Details

The MDS Coordinator will be responsible for the timely and accurate completion of both the RAI process and care management process from admission to discharge, in accordance with company policies, procedures, and federal, state, and certification guidelines. Responsibilities include managing Minimum Data Set, discharge and admission tracking, and coordinating information systems operations and education for the clinical department under the direction of the Director of Nursing and VP of Clinical Reimbursement.

Your Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate and manage daily stand-up meetings, review resident care, and set assessment reference dates.
• Ensure compliance with federal and state regulations regarding the RAI process.
• Monitor MDS and care plan documentation, ensuring documentation supports MDS coding.
• Maintain current MDS status for residents according to guidelines.
• Accurately enter resident data into computerized MDS programs.
• Complete precise MDS coding through medical record review, observation, and interviews.
• Perform other duties as assigned.

Your Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• At least 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations, and MDS 3.0 is preferred.
• Understanding of care planning and the Quality Indicator process.
