Who You Are:
We are seeking an (RAC-CT preferred) RN or LVN with a strong positive attitude toward customer service to act as an MDS Coordinator. This person will have a genuine interest in geriatric care and be able to motivate while maintaining relationship with staff and residents. The right candidate will always strive to do the best job possible, to be creative and to work towards accomplishing facility goals. This position will require the MDS Coordinator to be in the On-Call rotation one weekend per month.
Position Summary:
Coordinate the RAI process for all residents, performing/assisting as needed with assessments related to the process. Plan, organize, develop and maintain a complete MDS program, ensuring that all federal, state and local standards are adhered to . Assist in indicator development, standards development, data collection, display and analysis, problem solving and reporting.
Essential Functions:
Coordinate the IDT for Care Planning, assuring the highest quality of care for residents is developed at and above CMS guidelines.
Timely MDS completion per RAI manual. Giving special attention to completion of forms.
Timely completion of RAI process and RAP summary. Identification of problems, goals, and approaches.
Proper and accurate response to regulation changes and evaluation of the Care Plan system.
Assure timely signatures are obtained.
Effectively communicate with nurses and IDT on the current status of Care Plans.
Responds timely and in writing to deficiencies in MDS audits.
Assist with admission process.
Consistently demonstrate hospitality behavior and the core values of quality, respect, service and integrity.

Qualifications
Minimum Licensed Vocational Nursing degree (it can be an RN or LVN). MDS experience required. Preferred RAC - CT, Two years of experience working as a licensed vocational nurse including some medical/surgical experience and awareness of the needs of geriatric residents.
