Job summary

Aviata at South Daytona is seeking an RN or LPN MDS Coordinator to join our care team. You will oversee the Resident Assessment Instrument (MDS, CAA’s, and Care Plan) to ensure accuracy and compliance with Federal and State regulations in a supportive, family-oriented environment.

This role emphasizes collaboration with facility staff, ongoing professional development, and a commitment to compassionate, high-quality resident care.

Qualifications

• Current, unencumbered state license to practice as an RN or LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• Compassionate and dedicated to patient care.

Responsibilities
