Job summary

Westbury Center of Jackson is seeking a valued RN/LPN MDS Coordinator to ensure our residents receive the highest level of care while working in a supportive and fulfilling environment.

As the MDS Coordinator, you will oversee RAI/MDS assessments in accordance with CMS and LTCI guidelines, collaborate with nursing and other disciplines, and prepare QM/QI reports from MDS data to support quality care metrics.

Experience with MDS processes and regulatory compliance is required.

Qualifications

• RN or LPN licensed professional with knowledge of MDS processes.
• Experience coordinating MDS/RAI in long-term care settings.
• Ability to collaborate across disciplines to improve care plans.

Responsibilities

• Review and process MDS assessments per CMS and LTCI guidelines.
• Collaborate with nursing staff and other disciplines to ensure care-plan goals.
• Prepare QM/QI reports from RAI/MDS data and ensure timely transmission.

MDS Coordinator Summary

As the MDS Coordinator, you'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.

MDS Coordinator Responsibilities

Your impactful contributions will include:

• Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals
• Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS
• Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data
