Job summary

Lake City Center for Nursing and Healing in Morrow, GA, is seeking an experienced MDS Coordinator to oversee resident assessments and ensure accuracy, quality care, and compliance with CMS and long-term care guidelines. You will collaborate with nursing staff and department heads, prepare QM/QI data for performance improvement, and ensure timely submission of MDS procedures.

Qualifications

• Requires RN license (GA) with long-term care experience.
• Experience with MDS/RAI assessment preferred.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.

Responsibilities

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation

What You’ll Do

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines
