Aviata at San Jose in Jacksonville, FL seeks a qualified MDS Coordinator (RN or LPN) to oversee the Resident Assessment Instrument process and ensure compliance with Federal and State Regulations. You will coordinate MDS, CAA's and Care Plans in partnership with the care team.

The role emphasizes compassionate, detail-oriented care and timely documentation. Six months of MDS experience preferred; RAC-CT or RNAC is a plus.

Qualifications

• Must hold an active RN or LPN license.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six (6) months of experience as a MDS Coordinator or similar role preferred.

Responsibilities

• Oversee the coordination and completion of the MDS, CAA's and Care Plan in accordance with Federal and State Regulations.
• Monitor and document the management of Medicare/ managed care residents.
• Provide innovative, responsible healthcare improvements to systems and processes.

Job Summary

We are seeking a qualified MDS Coordinator (RN or LPN) to join our team! As a key member of our healthcare staff, you will be responsible for overseeing the coordination and completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations. Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an RN or LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six (6) months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
