Job summary

Forest Park Nursing Center is seeking an experienced MDS Coordinator to oversee RAI and care management from admission to discharge, under the direction of the DON and VP of Clinical Reimbursement. You will ensure compliance with Federal, State, and Certification guidelines and coordinate data systems for the clinical department.

The role requires RN/LVN licensure, at least 2 years in a Skilled Nursing Facility, and expertise in MDS 3.0, PPS, and OBRA.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations and Minimum Data Set.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS completions based on clinical criteria in the RAI User’s Manual.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Comply with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS programs accurately and timely.
• Complete coding of the MDS using chart review, observation, and interviews.
• Other duties may be assigned.
