Job summary

Hillcrest of North Dallas is seeking a qualified MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge, following company policy and federal/state guidelines. Under direction from leadership, may coordinate education for the clinical department.

The role requires an RN license and at least 2 years in a Skilled Nursing Facility, with strong knowledge of MDS, Case-Mix, and Medicaid reimbursement.

Qualifications

• Graduate of an approved RN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process; OBRA regulations and Minimum Data Set.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on need for additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations for MDS and care planning processes.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per guidelines and ensure data entry into MDS systems.
• Code MDS accurately from medical records and interviews with staff, residents, and families.
• Attend interdisciplinary meetings to communicate changes and update records.
• Assist with changes in physician orders and ensure MDS reflects these changes.
• Prepare scheduling and notices for care planning conferences; communicate outcomes to staff and residents.
• Continuously update knowledge on data entry and computer technology.
• Submit electronic documentation to state databases per policy.
• Complete final MDS and submit data to state/federal agencies.
