Citizens Memorial Hospital in Bolivar, Missouri is hiring an RN or LPN to join Parkview Healthcare Facility as an MDS Coordinator/Resident Care Coordinator, leading with skill and compassion.

You will complete auditing and submission of MDS 3.0, coordinate with IDT to plan care, review records, and ensure resident outcomes.

The role emphasizes attention to detail, LTC/MDS experience, and appropriate licensure (LPN/LVN/RN).

Qualifications

• Education: LPN certification/Associates or Bachelor’s degree in Nursing.
• Experience: LTC and MDS experience preferred.
• Licensure: LPN, LVN, RN.
• Physical Requirements: Sit or stand for extended periods and move throughout the facility.

Responsibilities

• Completing, auditing, and submitting Medicare, Medicaid, and PPS assessments (MDS 3.0) according to regulatory timelines.
• Collaborating with the Interdisciplinary Team (IDT) to develop and update individualized, comprehensive care plans.
• Reviewing clinical records and nursing documentation to ensure quality resident outcomes.
