Job summary

Spring River Rehabilitation and Care Center in Roswell, NM is hiring an MDS Coordinator (Minimum Data Set). This role leads the RAI process, ensures accurate MDS submissions, and drives reimbursement integrity through PDPM coding and regulatory compliance.

The ideal candidate is an organized, clinically knowledgeable nurse with strong data-driven decision-making and collaboration across disciplines to improve resident outcomes.

Qualifications

• Active RN or LPN license in NM.
• Minimum 2 years’ nursing experience, preferably in long-term care.
• MDS experience preferred; strong understanding of MDS 3.0, PDPM, and RAI process required.
• Knowledge of Medicare/Medicaid reimbursement and Quality Measures.
• Experience with audits, reporting, and regulatory compliance preferred.
• Proficient in EHR and MDS software systems.
• Strong organizational, analytical, and communication skills.
• Ability to work independently while collaborating across departments.
• High attention to detail and commitment to clinical accuracy.

Responsibilities

• Oversee and manage the RAI process, including MDS assessments, CAAs, and interdisciplinary care plans.
• Ensure accurate and timely completion and submission of all MDS assessments in CMS and state regulations.
• Audit clinical documentation to support accurate coding and reimbursement (Outcome Oriented).
• Maintain ARD scheduling and ensure adherence to regulatory timelines.
• Ensure care plans accurately reflect resident needs and services provided.
• Support appropriate Medicare and Medicaid reimbursement through accurate PDPM coding.
• Collaborate with billing, therapy, and finance teams to ensure alignment and accuracy.
• Stay current on CMS regulations, PDPM updates, Quality Measures, and survey readiness.
