Job summary

The Boulevard Post Acute is seeking an experienced MDS Coordinator to join our on-site team in Virginia. You will ensure timely, accurate MDS assessments, maintain compliance with federal/state regulations, and support care planning through data-driven reporting.

Ideal candidates hold an active RN or LPN license with minimum 3 years in long-term care and hands-on MDS/RAI experience. Competitive hourly pay and full benefits accompany a stable FT schedule.

Qualifications

• Graduate of an accredited School of Nursing (RN, BSN, or LPN).
• Current, active RN or LPN license required.
• Minimum of three (3) years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail and accuracy.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments in accordance with regulatory requirements and deadlines.
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes for accuracy and timeliness.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership team.
• Provide feedback to clinical teams on documentation and care planning.
• Participate in surveys, audits, and regulatory reviews with required documentation.
• Assist with audit responses and Medicare/Medicaid compliance.
• Stay current on Medicare/Medicaid/RAI/MDS regulations and guidelines.
• Support MDS-related quality improvement initiatives.
• Collaborate with interdisciplinary teams to accurately represent resident care needs.
