Job summary

River Grove Health and Rehab in Tennessee is seeking an LPN or RN to fill the MDS Coordinator role on-site. You will work with an interdisciplinary rehab team to ensure accurate MDS/RAI assessments, chart reviews, and timely documentation in accordance with federal and state guidelines.

The ideal candidate holds an active RN or LPN license, MDS certification, CPR, and at least three years of clinical experience, preferably in long-term care, with strong knowledge of MDS 3.0.

Qualifications

• Active RN/LPN license
• Certification in MDS
• Current CPR certification
• Experience with MDS 3.0
• Minimum three years of clinical experience; long-term care setting preferred

Responsibilities

• Ensure interdisciplinary rehab team process in MDS/RAI formulation.
• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
• Verify all MDS documents required either by Federal and/or State Guidelines or Organizational policy are easily available at all times is completed timely as set forth by Federal Regulations.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Assist with the coordination and participate in the completion of the Resident Assessment Instrument in accordance with current Federal and State Regulations.
