Memorial Care Center in Illinois is seeking an RN to manage MDS, admissions, and care plan processes in a long-term care setting. You will collaborate with physicians, residents, and staff to ensure accurate assessments and timely CMS reporting.

The role emphasizes teamwork and adherence to CMS timelines. The position requires an accredited nursing program graduate and an active RN license in the state, with prior long-term care/MDS experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state you will be working.
• Two years previous experience with long-term care plans, MDS regulations, and CMS related to MDS preferred.
• Excellent communication skills to interact verbally with residents, visitors, and staff.
• Ability to manage time and organize daily schedule to meet productivity standards.
• Ability to stand and walk for long periods of time.
• Ability to lift heavy objects using proper lifting techniques and with assistance as necessary.
• Manual dexterity and coordination for patient care and equipment.
• Ability to perform documentation electronically and manually.
• Ability to work independently with minimal supervision.
• Must be a team player.

Responsibilities

• Completes MDS, admission, significant change, and other CMS-related forms.
• Enter MDS data into the patient information system and transmit to CMS per CMS timelines.
• Ensure MDS/CAA records are available for a 15-month period and coordinate with the interdisciplinary team.
• Develops annual care plan goals coordinating with team members.
