Chatham Hills Subacute Care Center in New Jersey is seeking an experienced MDS Coordinator to join our clinical leadership team. This role focuses on accuracy of MDS assessments, regulatory compliance, and resident-focused care within a collaborative interdisciplinary environment.

The ideal candidate holds an active NJ RN or LPN license, has MDS 3.0/RAI knowledge, and can lead care plan meetings, ensure timely submissions, and educate staff on documentation requirements.

• Active NJ RN or LPN license.
• Experience with MDS in a skilled nursing facility.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and CMS regulations.
• Experience coordinating interdisciplinary care plans.

• Ability to meet deadlines and manage multiple priorities.

This is an excellent opportunity for a dedicated nursing professional who understands the MDS process and is committed to ensuring accurate assessments, quality resident care, and regulatory compliance.

Responsibilities

• Coordinate and oversee the facility's MDS assessment process in accordance with CMS guidelines and regulatory requirements.
• Ensure timely and accurate completion and submission of MDS assessments.
• Collaborate with nursing, rehabilitation, social services, dietary, and other interdisciplinary team members.
• Lead and coordinate interdisciplinary care plan meetings.
• Monitor assessment schedules, care plans, and supporting documentation for accuracy and completeness.
• Ensure appropriate utilization of the RAI Manual, MDS 3.0, and PDPM guidelines.
• Identify opportunities to improve clinical outcomes, documentation, and reimbursement accuracy.
• Maintain compliance with federal and state regulations and participate in audits and surveys.
• Provide guidance and education to staff regarding MDS documentation and assessment requirements.

Qualifications

• Current, active RN or LPN license in the state of New Jersey.
• Previous MDS experience in a skilled nursing facility required.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and CMS regulations.
• Experience coordinating interdisciplinary care plans.
• Excellent organizational, communication, and time-management skills.
• Ability to meet deadlines and manage multiple priorities independently.
