Qualifications

• Must hold an active and unrestricted RN or LPN license.
• Minimum of 2 years of MDS experience in long-term care or skilled nursing.
• Strong knowledge of the MDS/RAI process and regulatory requirements.
• Experience with care planning and clinical documentation; RAC-CT preferred.

Responsibilities

• Coordinate timely and accurate completion of MDS assessments.
• Oversee the RAI process and ensure regulatory compliance.
• Review clinical documentation for accuracy and completeness.
• Manage MDS transmissions and required submissions.
• Collaborate in interdisciplinary care planning.
• Monitor Quality Measures and identify improvement opportunities.
• Ensure care plans reflect individual needs and goals.
• Support accurate reimbursement through proper assessment and documentation.
