The ideal candidate holds an active Florida RN or LPN license, has at least 2 years in a skilled nursing setting, and can coordinate interdisciplinary teams while meeting deadlines.

Qualifications

• Active Florida RN or LPN license required.
• Minimum of 2 years of Skilled Nursing Facility (SNF) experience.
• Strong knowledge of MDS, Medicare regulations, and the care-planning process.
• Experience coordinating interdisciplinary care planning and resident assessments.
• Excellent communication, organizational, and leadership skills.

Responsibilities

• Oversee resident care quality to meet standards and regulatory requirements.
• Conduct comprehensive resident evaluations to inform care planning.
• Develop and update individualized, goal-oriented care plans.
• Coordinate interdisciplinary team meetings with nursing, therapy, dietary, social services, families.
• Ensure timely completion of MDS assessments and documentation.
• Monitor MDS and care-planning schedules for non-Medicare residents.
• Maintain compliance with Medicare, MDS, and SNF regulations.
• Provide education on MDS, RAPS, Triggers, and care planning to staff.
• Collaborate with nursing leadership to improve resident care and outcomes.

Pay Rate: $38 – $44 per hour

Space Coast Healthcare and Rehabilitation is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our leadership team. This position plays a critical role in ensuring accurate assessments, comprehensive care planning, regulatory compliance, and positive resident outcomes.

Key Responsibilities

• Oversee and monitor resident care quality to ensure services meet established standards and regulatory requirements.
• Conduct comprehensive resident evaluations and interviews to identify individual needs and support accurate care planning.
• Develop, maintain, and update individualized, goal-oriented care plans focused on resident outcomes and quality of life.
• Coordinate and facilitate interdisciplinary team meetings with nursing, therapy, dietary, social services, caregivers, and families.
• Ensure accurate and timely completion of MDS assessments and related documentation.
• Monitor MDS and care-planning schedules, including requirements for non-Medicare residents.
• Maintain compliance with applicable Medicare, MDS, and skilled nursing facility regulations.
• Provide education and in-service training to staff regarding MDS, RAPS, Triggers, care planning, and related requirements.
• Collaborate with nursing leadership and other departments to identify opportunities for improved resident care and outcomes.
• Participate in nursing management meetings and contribute to effective communication and teamwork.
• Establish and maintain professional relationships with healthcare providers, specialists, and community partners as needed.

Qualifications

• Licensure: Active Florida RN or LPN license required.
• Experience: Minimum of 2 years of Skilled Nursing Facility (SNF) experience required.
• Strong knowledge of MDS, Medicare regulations, and the care-planning process.
• Experience coordinating interdisciplinary care planning and resident assessments.
• Excellent communication, organizational, time-management, and leadership skills.
• Strong attention to detail and ability to meet regulatory deadlines.
• Ability to work collaboratively with residents, families, nursing staff, and interdisciplinary team members.
