Job summary

Melbourne Healthcare and Rehabilitation in Melbourne, FL seeks an experienced MDS Director (RN/LPN) to lead the facility’s MDS program, ensuring accurate assessments and compliant documentation. The role oversees care planning, regulatory compliance, and appropriate reimbursement while coordinating with an interdisciplinary team to promote excellent resident outcomes.

Ideal candidate has MDS 3.0/PDPM knowledge, strong leadership, and 2+ years of MDS coordination in skilled nursing.

Qualifications

• Active, unencumbered Florida RN or LPN license.
• Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility.
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning.
• Strong leadership, organizational, communication, and time-management skills.
• Ability to manage multiple deadlines while maintaining accuracy and regulatory compliance.
• Experience working effectively with interdisciplinary teams.

Responsibilities

• Oversee the accuracy, completeness, and timeliness of all MDS assessments and supporting documentation.
• Ensure compliance with applicable state and federal regulations, including Medicare and PDPM requirements.
• Conduct comprehensive resident assessments and interviews.
• Develop, implement, and monitor individualized care plans for short-term and long-term residents.
• Lead and participate in interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, specialists, residents, and families to support positive resident outcomes.
• Provide leadership, guidance, and education to nursing staff.
• Conduct in-service training related to MDS, RAPS, triggers, PDPM, Medicare guidelines, and care planning.
• Maintain and manage MDS and care plan schedules for all residents.
• Monitor assessment and documentation processes to ensure regulatory compliance and timely completion.
• Partner with facility leadership and interdisciplinary departments to identify opportunities for improved clinical outcomes and documentation.

Schedule: Full-Time | Tuesday–Friday | 10-hour shifts | 6:00 AM–4:30 PM
Compensation: $83,200–$95,680 per year, based on experience
