Position: MDS Coordinator

Location: Boynton Beach, FL

Shift: Full-Time | 8:30 AM – 5:00 PM

Salary: $38- $46

About The Role

Heartland Nursing and Rehabilitation Center is seeking a skilled and compassionate MDS Coordinator to join our team. In this role, you will play a key part in ensuring that our long-term residents receive the highest standard of care through detailed assessments, individualized care plans, and strong collaboration with our interdisciplinary team.

Key Responsibilities

• Oversee care standards to guarantee exceptional support and attention for all residents.


• Conduct thorough assessments and interviews for new residents to develop personalized care plans.


• Design and implement care plans with measurable goals to promote both short-term and long-term resident improvement.


• Facilitate interdisciplinary meetings with healthcare professionals, families, and caregivers for a holistic approach to resident care.


• Partner with specialists to achieve the best possible outcomes for residents.


• Lead nursing management meetings and provide in-service training on MDS, RAPS, and Triggers to keep staff informed and proactive.


• Ensure compliance by managing the MDS/Care Planning schedule for non-Medicare residents and meeting all deadlines and standards.

Qualifications

• Licensed Registered Nurse (RN) in the state of Florida


• Minimum of 2 years of experience in a skilled nursing facility (SNF)


• Strong knowledge of MDS, Medicare regulations, and care planning processes
