Bethesda Health Group in Belleville, IL seeks a Registered Nurse to perform MDS, admission and care planning activities for long-term residents. You will coordinate with physicians and the interdisciplinary team to optimize resident care and ensure CMS timelines are met.

You will maintain thorough documentation, contribute to annual goals, and support residents and families with compassionate, professional care within a supportive team environment.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state you will be working.
• Two years previous experience with long-term care plans, MDS state regulations, and CMS related to MDS preferred.

Responsibilities

• Completes all MDS, admission, significant change, change of therapy, end of therapy, discharge and re-entry tracking forms and quarterly/annual assessments.
• Ensures MDS data is entered into the patient information system and transmitted to CMS per CMS timelines.
• Communicates with physicians, team members and residents to maintain accurate assessments and care plans.

Summary

Completes all MDS, admission, significant change, change of therapy, end of therapy, discharge and re-entry tracking forms as well as quarterly and annual assessment using directions provided in the RAI manual. These directions can vary from year to year. Enters appropriate MDS data into a patient information system and transmits to CMS via the state agency according to the timelines set forth by CMS. Have all MDS/CAA records available for a fifteen (15) month period. Determines that all individuals involved in the RAI process complete their specific MDS sections, CAAs and Care Plans according to the timeframe regulated by CMS. Communicates with physicians, team members and all involved individuals to maintain accurate assessments through interaction and routine meetings. Provides input into the care plan process initially and throughout the resident’s stay. Collaborates with the interdisciplinary team for problem solving and any resident-related issues that will enhance the care provided to each resident. Develops and implements specific goals on an annual basis that will coordinate with those of all team members. May be assigned other duties as appropriate.

Job Qualifications

• Graduate of an accredited nursing program

• Excellent communication skills to interact verbally with residents, visitors, and staff
• Ability to manage time and organize daily schedule to meet productivity standards

• Manual dexterity, coordination and skillful use of hands when working with patients and equipment
• Ability to physically perform necessary documentation both electronically and manually
• Ability to work independently with minimal supervision
• Must be a team player

Pay range: $63,943 - $106,407 annually
