Job summary

The Orchards Michigan in Redford Township, MI is seeking an MDS Coordinator to join our clinical team and oversee accurate MDS assessments and care plans for residents.

This role requires an active RN or LPN license, long-term care experience, and strong organization, communication, and computer skills to maintain compliant records and support quality care initiatives.

You will collaborate with the DON and participate in QAPI and regulatory activities to improve resident outcomes.

Qualifications

• Must have valid RN or LPN license.
• Must have prior MDS experience.
• Long-term care experience required.
• Must be able to work 40 hours per week.
• Highly organized, professional, and motivated.
• Solid computer skills.
• Excellent communication skills.
• Friendly and a team player.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.
