Job summary

The Orchards is seeking an MDS Coordinator to join our team. The role focuses on completing accurate MDS assessments and care plans, initiating plans, and distributing monthly calendars in a timely manner.

Qualified candidates hold an RN or LPN license, have prior MDS experience in long-term care, and are highly organized with strong communication skills. This 40-hour-per-week position offers comprehensive benefits and a collaborative work environment.

Qualifications

• Valid RN or LPN License.
• Knowledge of skilled nursing facilities survey process and regulations would be helpful.
• Must have prior MDS Experience.
• Long Term Care Experience Required.
• Must be able to work 40 hours a week.
• Must be highly organized, professional & motivated.
• Should have solid computer skills.
• Excellent communication skills.
• Should be friendly and a team worker.

Responsibilities

• Monitoring & auditing clinical records, ensuring accuracy & timeliness.

• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.
