Job summary

Aviate in Piscataway is seeking a full-time MDS Coordinator RN to oversee the Resident Assessment Instrument (RAI) process, ensure accurate MDS assessments, and develop resident-centered care plans that support quality care and appropriate reimbursement.

The role requires an active NJ RN license, at least 1 year as an MDS Coordinator in a skilled nursing facility, and strong knowledge of CMS guidelines, PDPM, RAI, with proficiency in PointClickCare.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 1 year experience as an MDS Coordinator in a skilled nursing facility (SNF).
• Strong knowledge of the RAI manual, CMS guidelines, and PDPM.
• RAC-CT certification preferred but not required.
• Proficiency with EHR software (PointClickCare).
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and oversee the timely completion of the MDS assessment process (PPS and OBRA).
• Collaborate with the IDT to develop and update resident-centered care plans.
• Review clinical documentation to ensure accurate reflection of care and MDS coding.
• Monitor and manage the MDS transmission validation to the state database.
• Analyze QM data to identify clinical improvement opportunities and support QAPI initiatives.
• Stay updated on CMS guidelines, PDPM reimbursement structures, and Medicaid regulations.
