Job summary

August Healthcare seeks a clinically strong MDS Coordinator (RN) to lead the resident assessment process in a full-time, day shift role. You will manage the RAI/MDS, CAAs, and care planning across payer sources, ensuring accuracy, compliance, and timely MDS transmission.

Qualifications

• Active RN license in North Carolina.
• Minimum 3 years clinical experience; long-term care preferred.
• Strong knowledge of RAI/MDS regulations and reimbursement systems.
• Excellent analytical, organizational, and problem-solving skills.
• Ability to lead interdisciplinary collaboration and meet regulatory deadlines.

Responsibilities

• Oversee the full RAI/MDS, CAA, and care planning process.
• Ensure assessments reflect resident condition, services, and needs.
• Lead PPS, Medicare, and utilization review meetings.
• Ensure timely completion, validation, and transmission of MDS.
• Collaborate with therapy, nursing, and interdisciplinary teams on ARDs.
• Monitor Quality Measures and drive performance improvement.
• Educate staff on the RAI process.
• Prepare documentation for audits, surveys, regulatory reviews.
• Participate in quality and workflow process improvements.
