Job summary

Cambridge Rehabilitation and Nursing Center is actively seeking qualified nursing professionals for full-time, part-time, and per diem RN/LPN roles in Cambridge, MA. The position focuses on completing MDS assessments, creating and updating care plans, and coordinating with nursing, rehabilitation, dietary, and social services teams to ensure optimal resident care.

The role requires prior long-term care experience, knowledge of MDS and PDPM, and strong documentation skills.

Qualifications

• Graduate of an accredited school of nursing for RN or LPN licensure.
• One year experience in long-term care environment.
• Knowledge of MDS and Care Plan process; PPS and PDPM knowledge.

Responsibilities

• Schedules and completes MDS assessments and care plans.
• Monitors MDS completion by other disciplines and signs off on RN completion.
• Initiates care plans to achieve best outcomes for residents.
• Conducts care plan conferences for assigned residents.
• Maintains and updates all care plans and assessments quarterly.
