Job summary

American Senior Communities is seeking an MDS Coordinator/Medical Records professional for Summit City Nursing in Fort Wayne, IN. The role centers on coordinating RAI processes, leading interdisciplinary care planning, and ensuring EMR utilization in collaboration with care teams.

The ideal candidate is an RN with management experience in long-term care, comfortable with regulatory requirements and eager to contribute to clinical excellence within a patient-centered environment.

Qualifications

• RN graduate from an accredited school of nursing; BSN preferred.
• Minimum of one year in nursing management in long-term care.
• Two years of professional nursing experience in long-term care, acute care, restorative care, or geriatric settings.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) and interdisciplinary care planning while ensuring regulatory compliance.
• Ensure successful utilization of the EMR through auditing, analysis, and training.
• Collaborate with physicians, office staff, nursing management and staff to optimize resident care and documentation.
