Job summary

Avalon Health Care Inc. is seeking an experienced MDS Nurse for a Resident Assessment Coordinator role. This full-time position requires managing Medicare and managed care residents from admission through discharge and ensuring OBRA assessments are completed accurately and on time.

The RAC will be a central member of the facility’s management team, coordinating care plans with the Resident Assessment Director. A current RN license, strong communication skills, and knowledge of RAI are essential.

Qualifications

• Active RN license in state of practice.
• Familiar with the RAI process and case management.
• Must be able to read, write, and speak English.
• Must pass health regulation and post-employment physical if required, including drug screening, background check and references.

Responsibilities

• Oversee Medicare and Managed Care residents from admission to discharge as directed by the Resident Assessment Director.
• Ensure timely completion of OBRA required assessments.
• Serve as a key financial and clinical member of the facility management team; not subject to nursing call schedules.

Schedule FT Mon - Fri Infrequent weekend rotation, estimated at one weekend every three months.

$44.00 - $48.83/hr

Responsibilities

The Resident Assessment Coordinator will be responsible for all of the facility Medicare and Managed Care residents as directed by the Resident Assessment Director. This will include the oversight of all service provided during inpatient stay from admission to discharge. This position is also responsible for the accurate and timely completion of all OBRA required assessments. This role is viewed as a key financial and clinical member of the management team. This position will not be subject to nursing “call” schedules.

Qualifications

• Must have a current, active license to practice as an Registered Nurse (RN) in State of practice.
• Must be well-versed in inter-personal communications.
• Must be familiar with the RAI process and Case Management.
• Must be able to read, write, and speak English.
• Must meet all local health regulation and pass post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation and reference inquiry.
