Responsibilities

• Complete the MDS, CAA’s, and care plans within regulated timeframes
• Coordinate scheduling the RAI process with the interdisciplinary team
• Assess residents through physical assessment, interviews, and chart reviews
• Discuss resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff
• Review information from hospitals, consults, and outside agencies for assessments and care planning
• Coordinate, identify, and initiate significant change MDSs
• Conduct PPS meetings, maintain MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Stay current with AANAC requirements

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
• AANC certification a plus. RAC-CT
• Knowledge of the RAI process and the Prospective Payment Process (PPS) is strongly preferred
• Experience as an MDS Nurse
