SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for care and services provided within the facility. The role involves conducting continual Minimum Data Set (MDS) reviews to optimize Resource Utilization Group (RUG) categories and overseeing the process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. The coordinator will integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure proper assessment and reimbursement.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Assess and determine the health status and level of care of all new admissions.
- Ensure the accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
- Communicate the level of care for new residents to all disciplines.
- Coordinate interdisciplinary participation in completing the MDS for each new admission according to regulatory time frames, ensuring completeness and thoroughness of documentation as mandated by federal, state, and medical standards.
- Maintain an accurate schedule of all MDS assessments with proper reference dates throughout the resident’s stay.
- Manage data entry to ensure accurate data entry and electronic submission of MDS assessments, verifying submissions, performing corrections when necessary, and maintaining records.
- Schedule and conduct resident care conferences in compliance with regulations, ensuring all MDS reviews are completed prior to conferences.
- Assist disciplines in formulating and revising care plans, ensuring problems are identified, prioritized, and goals are realistic, with appropriate nursing interventions.
- Evaluate resident care plans for comprehensiveness and individualization.
- Assess and ensure the achievement of desired outcomes, revising care plans as needed.
- Manage all level of care changes within the facility, notifying departments and transmitting forms to the appropriate agencies as required by law.
- Must be able to work on the floor as a nurse during emergency circumstances, such as staffing crises.
- Perform other duties as assigned.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.
- Registered Nurse with current unencumbered state licensure preferred.
- Long-term care experience preferred.
- LPN or RN licensure required.
- Ability to read, write, speak, and understand English.
