Job summary

Park Ridge Nursing and Rehabilitation Center is seeking an MDS/PDPM RN to plan, organize, and direct the MDS/PDPM process in accordance with federal and state standards. The role involves coordinating MDS schedules and collaborating with the interdisciplinary team to complete the Resident Assessment Instrument.

Requirements include an active Florida RN license, 1+ year nursing experience in a similar setting, and 6+ months as an MDS Coordinator. RAC-CT or RNAC is preferred.

Responsibilities

• Monitor and document management of Medicare and Managed Care residents with facility team members.
• Maintain and update schedules of MDS’s.
• Oversee coordination and participation in completing the Resident Assessment Instrument (MDS, CAA’s and Care Plan).
• Assist in completion of the Resident Assessment with the Interdisciplinary Team.
• Ensure MDS, RAI processes and Care plans are completed and transmitted timely.
• Assist in audits, denials and ADRs as requested by CES.
