Job summary

We seek a Registered Nurse to contribute to high-quality patient care, with responsibilities including MDS/RAI processes and care-plan coordination.

The role requires current RN licensure, experience in healthcare facilities, and knowledge of long-term care regulations.

Qualifications

• Must be a Registered Nurse licensed by the state.
• Experience in hospital, long-term facility, or other healthcare related facility is required.
• Prior experience completing MDSs and Care Plans is required
• Knowledge of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.

Responsibilities

• Assist in developing individualized patient care plans, identify patient needs and outcomes as required by Federal and State regulations.
• Contribute to the Resident Assessment Instrument (RAI) process following guidelines for completion, encoding and transmission of the MDS.
• Identify changes in patient condition with the interdisciplinary team and coordinate scheduling/completion of indicated assessments.
• Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff.
• Attend educational programs to maintain knowledge with current RAI/Prospective Payment System regulations.

Posted Salary Range: USD $43.00 - USD $44.00 /Hr.
