Job summary

Bluestone Health & Rehabilitation is seeking an experienced MDS Coordinator RN to join our nursing home team in West Virginia. You will oversee resident assessments, care planning, and coordination with interdisciplinary staff to ensure compliance and quality care.

Must be an RN licensed in the state, with recent MDS experience. Bachelor's degree in nursing is preferred, and RAC/CT certification is a plus. We offer competitive pay, PTO, benefits, and opportunities for growth.
