Job summary

Ascent Healthcare Management in Western North Carolina (Asheville, Hendersonville, Brevard, Salisbury, Rutherfordton) is seeking an MDS Coordinator.

You will manage MDS/RAI processes, ensure timely assessments, and support reimbursement while collaborating with nursing, therapy, and facility leadership. Ideal candidates are organized, clinically strong, and familiar with CMS rules and PDPM.

Qualifications

• Active RN or LPN license in good standing.
• Long-term care or skilled nursing facility experience required.
• Strong knowledge of MDS, care planning, and clinical documentation.
• Ability to meet deadlines with accuracy and strong communication.

Responsibilities

• MDS/RAI process — coordinate, complete, and schedule assessments per regulations.
• Coordinate care plans with interdisciplinary team and document changes.
• Support Medicare Part A documentation and reimbursement processes.
• Monitor quality measures and assist with audits and compliance.
• Serve as a resource on MDS, documentation, and care planning.
