Job summary

Spring River Rehabilitation and Care Center in Roswell, NM, seeks a detail-oriented MDS Coordinator (RN/LPN) to oversee the RAI process, ensure timely MDS assessments, and drive accurate coding for PDPM reimbursement. You will coordinate care plans with interdisciplinary teams and maintain CMS/regulatory compliance.

The role emphasizes clinical documentation accuracy, education, and collaboration across departments to support high-quality resident outcomes.

Qualifications

• Active RN or LPN license in New Mexico.
• Minimum 2 years’ nursing experience, preferably in long-term care.
• MDS experience preferred; strong understanding of MDS 3.0, PDPM, and RAI process required.
• Knowledge of Medicare/Medicaid reimbursement and Quality Measures.
• Experience with audits, reporting, and regulatory compliance preferred.
• Proficient in EHR and MDS software systems.

Responsibilities

• Oversee and manage the RAI process, including MDS assessments, CAAs, and interdisciplinary care plans.
• Ensure accurate and timely completion and submission of all MDS assessments in compliance with CMS and state regulations.
• Audit clinical documentation to support accurate coding and reimbursement.
• Maintain ARD scheduling and ensure adherence to regulatory timelines.
• Ensure care plans accurately reflect resident needs and services provided.
• Collaborate with billing, therapy, and finance teams to ensure alignment and accuracy.
