Adept of Omaha is seeking an organized, knowledgeable nursing professional to serve as our MDS Director. This position leads the coordination of the Minimum Data Set (MDS) and utilization management processes, supporting accurate assessments, individualized care plans, and quality resident care.
Coordinate and monitor the MDS assessment process to ensure accurate documentation and timely completion.
Maintain current knowledge of applicable federal and state regulations and professional standards.
Collaborate with the interdisciplinary team to develop comprehensive care plans that reflect each resident’s needs and goals.
Review care plans and nursing documentation to help ensure care is delivered as planned.
Monitor changes in resident condition and communicate the need for timely clinical follow-up.
Ensure MDS assessments and supporting documentation accurately represent resident status and meet regulatory and audit requirements.
Conduct regular documentation audits and follow up on identified concerns.
Assist nursing management with discharge planning and related documentation.
Complete required forms, reports, and administrative tasks.
Protect resident confidentiality, dignity, privacy, and rights.
Follow infection control and safety procedures and report hazards or equipment concerns.
Respond promptly and courteously to call lights when working in resident care areas.
Participate in departmental meetings, required training, and interdisciplinary communication.
Perform other related duties as assigned.
Current Nebraska LPN/RN license or authorization to practice in Nebraska.
Knowledge and training in all aspects of the MDS process.
One year of nursing experience in a long-term care setting preferred.
Strong computer, documentation, and organizational skills.
Ability to prioritize responsibilities and manage multiple deadlines.
Clear written and verbal communication skills.
Ability to work cooperatively with residents, families, nursing staff, and other departments.
