Job summary

ABHM | Maple Crest Health Center in Omaha, NE is seeking an experienced MDS Coordinator (RN) to oversee the interdisciplinary assessment and care planning process in our skilled nursing facility.

You will transmit MDS assessments, correct rejections, and ensure compliance with MDS/RAI requirements and federal/state regulations. A Nebraska RN license and at least 2 years of skilled nursing experience are required.

Qualifications

• Active Nebraska RN License (unencumbered).
• 2 years of skilled nursing experience (required).
• Strong critical thinking, independent judgment, and multitasking abilities in a fast-paced environment.
• Ability to read, write, and speak English.

Responsibilities

• Coordinate and oversee the full interdisciplinary assessment and care planning process in a skilled nursing facility.
• Transmit MDS assessments, correct rejected assessments, and adhere to MDS/RAI requirements.

What You'll Do:

• Coordinate and oversee the full collaborative, interdisciplinary assessment and care planning process in our skilled nursing facility
• Transmit MDS assessments, correct rejected assessments, and adhere to the Minimum Data Set (MDS) and resident assessment instrument (RAI) requirements
• Ensure all residents are assessed according to established timeframes or changes in status and in compliance with federal, state, and Medicare regulations

What You'll Need:

• ActiveNebraska RN License (unencumbered)
• 2 years of skillednurse experience (required)
• Strong critical thinking skills, independent judgment, and the ability to multitask and prioritize work assignments in a fast-paced work environment
