Job summary

Evergreen Retirement Community in the United States seeks a skilled Medicare Coordinator Registered Nurse to join the Health Center team. You’ll lead coordination of the Medicare program, ensure timely MDS documentation, and advocate for resident-centered care.

You’ll work 32 hours per week with a schedule: Mon, Wed, Fri 9am-4pm and Tue 11am-6pm, collaborating with Creekview Rehab and the leadership team to drive quality and operational excellence.

Responsibilities

• Coordinate and manage Medicare program for the Health Center.
• Complete accurate MDS documentation timely.
• Serve as a resource to unit staff promoting best practices and collaboration.
• Step in as a staff nurse when needed to ensure seamless care delivery.
• Assist in oversight of day-to-day operations and ensure high-quality service for residents.
• Participate in on-call rotation offering support, consultation and leadership coverage.
• Be an active member of the Creekview Leadership Team contributing to strategic planning and operational excellence.
