Job summary

Golden Sonora Care Center in Sonora, CA seeks an MDS Coordinator to assess new patients and transmit MDS forms for billing, ensuring compliant standards and high-quality resident care.

Ideal candidates hold an active RN/LVN license; DSD certification is preferred. Two years in post-acute or skilled nursing settings with supervisory/training experience are valued, with benefits like medical and PTO.

Qualifications

• Must have clear and active RN or LVN/LPN license in the state.
• DSD Certified preferred.
• Minimum 2 years in post-acute/skilled nursing/LTC or hospital.
• Experience in supervision and teaching principles/practice.
• Knowledge of nursing practices and applicable regulations.

Responsibilities

• Plan, develop, direct, evaluate and coordinate educational programs.
• Develop, evaluate and control quality of in-service programs.
• Maintain records, manuals and instructional materials.
• Complete forms, reports and attendance/records.
• Provide written and oral reports to administrator on training.
• Collaborate with business office on employee benefits.
