Mulberry Health and Rehab in Franklin, TN is hiring an MDS Coordinator on a part-time basis (20 hours). The role requires an active RN license and may include an LPN license with relevant MDS/RAI experience.

You will lead the MDS/RAI process, coordinate care planning, and ensure timely documentation to support resident outcomes. Join our compassionate, resident-centered facility and contribute to quality care, compliance, and teamwork.

Qualifications

• Current RN license in good standing.
• Current LPN license with experience.
• Prior MDS/RAI experience preferred (but willing to train the right candidate!)

Responsibilities

• Lead and manage the MDS/RAI process from start to finish.
• Coordinate interdisciplinary care planning.
• Ensure accurate and timely completion of assessments and documentation.
• Collaborate with nursing leadership to maintain regulatory compliance.
• Support staff with education related to MDS processes and quality measures
