Mulberry Health and Rehab in Franklin, TN is seeking an MDS Coordinator to oversee the MDS/RAI process and drive high-quality resident care. You will coordinate interdisciplinary care planning and ensure accurate assessments while maintaining regulatory compliance in a skilled nursing setting.

The ideal candidate holds an RN license, with LPN experience and MDS/RAI familiarity. This role offers PT/FT options and a supportive leadership team committed to improving resident outcomes.

Qualifications

• Registered Nurse with valid licensure and knowledge of MDS/RAI processes.
• Experience in long-term care and regulatory compliance preferred.
• Strong organizational and documentation skills.

Responsibilities

• Lead and manage the MDS/RAI process from start to finish.
• Coordinate interdisciplinary care planning.
• Ensure accurate and timely completion of assessments and documentation.
• Collaborate with nursing leadership to maintain regulatory compliance.
• Support staff with education related to MDS processes and quality measures.
