The Boulevard Post Acute in Fairfax, VA is seeking an MDS Coordinator for a full-time, on-site role. You will lead timely MDS assessments, ensure compliance with Medicare/Medicaid regulations, and collaborate with the care team to support resident planning and reimbursement.

Ideal candidates hold an active RN or LPN license and have 3+ years in long-term care with MDS/RAI experience. We offer daily pay and a comprehensive benefits package with tuition reimbursement and retirement options.

Formación

• MDS assessments must be completed timely and accurately.
• Compliance with Medicare/Medicaid regulations related to MDS documentation.
• Supervise data entry, validation and transmission of MDS data.
• Identify and resolve data discrepancies and validation issues.
• Prepare MDS-related reports for DON and leadership.
• Provide feedback to clinical teams on documentation and care planning.
• Participate in surveys, audits and regulatory reviews, providing required documentation.
• Assist with audit responses and Medicare/Medicaid compliance.
• Stay current on changes to Medicare, Medicaid, and RAI/MDS regulations.
• Support quality improvement initiatives related to MDS.

• Collaborate with interdisciplinary teams to accurately represent resident care needs.
