Pacific Coast Manor in Capitola, CA seeks an MDS Coordinator to oversee timely, accurate patient assessments across departments, ensuring compliance with applicable laws and policies.

Position requires a California LVN license (RN preferred) and 2 years of nursing or 1 year of MDS experience, with strong communication and organizational skills. The role includes education of staff, care plans oversight, and collaboration with billing and DON.

Qualifications

• Current California LVN license required; RN license may be preferred.
• Two years of nursing experience or one year of MDS experience preferred.

Responsibilities

• Coordinate and assist with completion and submission of accurate, timely patient assessments per requirements and policies.
• Report any changes in a patient’s condition to the Director of Nursing (DON).
• Educate direct care associates on patient documentation, updates, and regulations.
• Ensure care plans are comprehensive and meeting patient needs.
• Review Final Validation Reports confirming assessments are completed and accepted into CMS and other systems prior to billing.
• Collaborate with Business Office regarding billing decisions.
• Continuously review CMS Reports to address issues and improve processes to ensure timely submissions.
• Attend Daily PPS Meetings and Monthly Triple Check.
• Performs staff nurse functions when necessary.
• Lead by example with kindness and professionalism.

Responsibilities:

• Coordinates and assists with completion and submission of accurate, timely, inter-departmental, patient assessments, in accordance with all requirements and policies
• Reports any changes in a patient’s condition to the Director of Nursing (DON)
• Provides education to direct care associates regarding patient documentation, overseeing updates or changes, and training on Skilled Nursing Facility and other Regulations
• Ensures care plans are comprehensive, accurate and meeting patient needs
• Reviews Final Validation Reports confirming assessments have been completed and accepted into the CMS and other systems as necessary and prior to billing
• Collaborates with Business Office regarding when to/when not to bill
• Continually reviews CMS Reports to address issues and improve processes to ensure that assessments are accurate and submitted on time
• Attend and participates in the Daily PPS Meeting, Monthly Triple Check
• Performs functions of a staff nurse when necessary
• Leads by example in treating everyone at work kindly and professionally
