Job summary

Halfway to Heaven Nursing Home seeks an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. You will coordinate the RAI process, assure timeliness and completeness of the MDS, CAAs, and the interdisciplinary care plan.

The role requires RN/LVN certification with 2+ years of nursing experience in skilled nursing settings and strong knowledge of Medicare PPS, Medicaid reimbursement, OBRA and

Qualifications

• Knowledge of Case-Mix and Federal Medicare PPS process.
• Understanding of Medicaid reimbursement requirements.
• Familiarity with OBRA regulations and MDS processes.

Responsibilities

• Assist the Director of Nursing with ensuring documentation meets guidelines.
• Coordinate the RAI process to ensure timely and complete MDS, CAAs, and care plans.
• Participate in regulatory surveys and corrective actions.
• Monitor validation of MDS submissions and maintain records.
• Ensure accurate Assessment Reference Date and relevant data capture.
