Job summary

Colonial Pines Healthcare Center in San Augustine, TX is seeking an experienced MDS Coordinator (LVN) to manage Medicaid/Medicare reimbursement through MDS 3.0, care plans, and regulatory documentation.

You will coordinate the MDS/Care Plan schedules, ensure timely submissions, and support the interdisciplinary team while maintaining high quality patient care and accurate billing. Experience in RAI/PPS processes and strong attention to detail are essential.

Qualifications

• Current LVN license in the state of Texas.
• 2+ years of experience in a skilled nursing facility with MDS coordination.
• Demonstrated proficiency in the RAI / PPS processes.
• Strong attention to detail, documentation, and resident assessment skills.
• Good communication, clerical, organizational, and computer skills.

Responsibilities

• Maintain MDS/Care plan schedule for interdisciplinary team.
• Coordinate preparation of MDS and Care Plan per regulatory requirements.
• Track Medicare resident benefit days and coordinate billing data with management.
• Complete responses to requests for information and appeals within time limits.
• Prepare Medicare denial letters for residents discharged from Medicare coverage.
• Support Medicaid reimbursement documentation.
