Reunion Plaza Senior Care and Rehabilitation Center in Texarkana, TX seeks an MDS Coordinator (LVN) to oversee Medicaid/Medicare reimbursements, MDS/Care plans, and regulatory compliance. You will support patient care delivery and ensure high-quality, compassionate services in a senior care setting.

The role emphasizes accurate documentation, interdisciplinary collaboration, and timely handling of Medicare/Medicaid processes to optimize reimbursement and care outcomes.

Qualifications

• MDS coordination and care planning experience is preferred.
• Must hold an active LVN license in Texas and familiarity with MDS 3.0 processes.
• Strong documentation, communication, and organizational abilities.

Responsabilités

• Maintain MDS/Care plan schedule for interdisciplinary team.
• Coordinate preparation of MDS and Care Plan per regulatory requirements.
• Track Medicare resident benefit days and coordinate with management for accurate billing.
• Respond to requests for information and appeals within time frames.
• Prepare Medicare denial letters for discharged residents as required.
• Assist in Medicaid documentation for reimbursement.
