Reunion Plaza Senior Care and Rehabilitation Center in Texarkana, TX is seeking an MDS Coordinator (LVN) to focus on Medicaid and Medicare reimbursement while coordinating MDS 3.0, care planning, and interdisciplinary collaboration. You will help ensure high-quality resident care in a compliant regulatory environment.

Qualifications

• 2+ years of experience in skilled nursing facilities with MDS/Care Plans.
• Proficiency in the RAI/PPS processes.
• Strong attention to detail and resident assessment skills.
• Good communication and clerical skills.

Responsibilities

• Maintain MDS/Care plan schedule for interdisciplinary team.
• Coordinate preparation of MDS and Care Plan as per regulatory requirements.
• Track Medicare resident benefit days and validate daily census for billing.
• Respond to information requests from intermediaries within required timeframes.
• Prepare Medicare denial letters for discharges from Medicare coverage.
• Participate in ongoing Medicaid reimbursement documentation reviews.
