Job summary

Aspire Senior Living is seeking a qualified MDS Coordinator (LPN or RN) to join our nursing team in Lincoln, NE. The role focuses on coordinating MDS assessments, care plans, and related documentation to ensure high-quality care for residents.

A $4,000 sign-on bonus is available for full-time joiners. The position requires current LPN or RN licensure (Missouri) and prior experience in long-term care or senior living, with strong geriatric clinical skills and excellent communication.

Qualifications

• Current LPN or RN licensure in the state of Missouri.
• Experience in a long-term care or senior living setting preferred.
• Strong clinical knowledge in geriatric care.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Coordinate the maintenance and completion of MDS assessments, care plans, and documentation for residents.
• Collaborate with nursing staff, healthcare providers, and residents to identify and address changes in resident conditions and needs.
• Develop and implement individualized care plans that meet residents' unique needs.
• Ensure accurate and timely completion of MDS assessments and care plans.
• Maintain accurate and up-to-date records and reports.
• Participate in quality improvement initiatives to enhance overall quality of care and services.
