Responsibilities

• Coordinate and conduct assessments to develop and implement comprehensive care plans with the healthcare team.
• Review and update MDS assessments, care plans, and resident records for accuracy and regulatory compliance.
• Collaborate with caregivers, physicians, and other professionals to provide high-quality resident care.
• Identify and report changes in residents' conditions, concerns, or complaints to the team and families.
• Develop and maintain effective communication systems to ensure coordinated care.
