Job summary

Beechwood Post Acute & Transitional Care in New London, CT, seeks an MDS Coordinator RN (LPN) to coordinate the MDS process and develop individualized care plans. You will collaborate with nursing staff, therapists, and other professionals to promote resident health and well-being.

Qualifications

• Current LPN licensure in the state of Connecticut.
• Experience in a post-acute care setting, preferably in an MDS Coordinator role.
• Demonstrated ability to work effectively in a fast-paced environment.
• Excellent communication and interpersonal skills.

Responsibilities

• Coordinate the MDS process, including assessing resident needs and developing care plans.
• Collaborate with the interdisciplinary team to identify resident needs and strategies.
• Ensure care plans reflect current needs and goals.
• Monitor resident progress and implement improvements as needed.
• Maintain accurate records of resident care and progress.
