Job summary

Complete Care - Connecticut is seeking an MDS Coordinator (RN or LPN) for a full-time day shift. The role requires strong MDS experience and knowledge of Case-Mix and Medicare PPS processes. You will monitor resident assessments, supervise input of MDS data, and coordinate care plan conferences.

Candidates must hold an active CT RN or LPN license. Long-term care experience is preferred. Join a compassionate team dedicated to quality care and resident well-being.

Qualifications

• MDS knowledge and Medicare PPS understanding required.
• Ability to coordinate care plans and conferences.
• Strong teamwork and communication skills.

Responsibilities

• Monitor and guide completion of resident assessments.
• Input and supervise MDS data entry.
• Transmit MDS assessments to state and obtain validation receipts.
• Schedule and lead care plan conferences quarterly and annually.
• Assist under the Director of Nursing and Administrator as needed.
